Instruction for completing the online milk order form:

1. Fill out online order form. Indicate how many milks you want per week on each line (not the number per day).
2. You will have the option to print the form once the order is submitted.
3. Send payment in an envelope to school. Checks made payable to St. William of York School. Lunch and Milk payments must be separate.
4. A separate order form needs to be submitted for each child, but you may send all payments in one envelope to school.

* If you make an error ordering your milk, contact the Milk Coordinator. Please do not submit an additional order.

**Payment is due by Friday, August 31, 2007 .
